Covid-19 Preparedness

To All Concerned:

As part of the COVID-19 Health Vaccination Program, the facility has partnered with CVS Pharmacy, the California Department of Public Health, and the Centers of Disease Control and Prevention (CDC) to provide vaccinations to our patients and staff. In preparation for our staff and residents to be vaccinated, we are working diligently to obtain consent from our patients and their representatives. You may receive a call from a facility staff member to obtain your consent. Once we have received consent, patients will be scheduled to receive the COVID-19 vaccination during the facility on-site administration clinic date. We anticipate the on-site clinic to take place in the very near future and will provide the date and time when we receive this information. We have also posted an electronic version of the consent forms needed along with the EUA FAQ Sheet for the Pfizer BioNTech COVID-19 Vaccine below. You may complete the appropriate consent form and send back via email to Dulce.odell@thespringshc.com.
